When can intercourse be resumed after penile implant surgery?
After the penile prosthesis implant surgery, the man can resume sexual activity after 4 to 6 weeks. This may vary depending on each patient and the type of surgery performed. This time of abstinence allows the tissues to heal, the wounds to heal and the corpora cavernosa to get used to the silicone rods and generate new vascular networks.
How is sex with a penile implant? Will I be able to have orgasms and ejaculate?
Thanks to the sophisticated design of the latest generation of penile prostheses and state-of-the-art surgical techniques, sexual intercourse with a penile prosthesis is now completely natural and with high satisfaction rates. In fact, very recent evidence (here you can read the Sexual Medicine Review paper) analyzing the statistics of penile prostheses has shown the following:

These are the main characteristics of sexual relations after penile prosthesis implantation:
- Orgasms: Having a penile prosthesis will not prevent a man from reaching the climax of the sexual act, on the contrary, when a man manages to recover erections thanks to the implant, he will also recover orgasms that he probably had not experienced for a long time.
- Ejaculation: No penile nerves or structures involved in ejaculation (prostate, seminiferous vesicles, ejaculatory ducts and urethra) are affected during surgery, therefore, ejaculation and reproductive capacity will remain intact.

Functioning of the hydraulic penile prosthesis
The hydraulic prostheses can be discreetly activated and deactivated, allowing men to have sex naturally and without interruption. The duration and rigidity of the erection is controlled by the amount of fluid injected into the cylinders, allowing men to maintain an erection for as long as they desire. This is how a hydraulic penile implant works:
- It consists of two silicone cylinders that are implanted in the corpora cavernosa of the penis and a fluid reservoir that is placed in the abdomen.
- The cylinders are connected to a piping system and a hand pump that is located in the scrotum.
- When the man desires an erection, he activates the hand pump, which causes fluid to be transferred from the reservoir to the inside of the cylinders, creating a rigidity similar to that of a natural erection.
- To deflate the prosthesis, the man presses a valve that releases the fluid back into the reservoir.
Functioning of the malleable or semi-rigid penile prosthesis
The malleable prostheses are simple and easy to use, and do not require a pump or to be activated, they allow sexual intercourse very similar to that of a physiological erection, but without detumescence or sudden loss of erection. The duration of the erection depends on how long the man wishes to keep the rods in erect position.
The great advantage of the penile implant over other treatments is that it is a definitive solution to the problem of impotence. Its useful life is very high, practically for almost all life and the more use it is given, contrary to what might be thought, the better, because it keeps the mechanism of the implant in proper functioning. According to a very recent article in the prestigious magazine Urology, there is evidence that inflatable prostheses have an average life of 20 years:

- The malleable prosthesis is composed of two flexible metal rods covered with silicone which, like the hydraulic ones, are implanted in the corpora cavernosa.
